The countless enemies in SACRED are divided into the following categories:
- Human enemies including Thieves, Robbers, Soldiers, etc.
- Animals including Wolves, Bears, Wild Boars, etc.
- Monsters including Giants, Dragons, Demons, Giant Spiders, etc.
- Goblinoids including Goblins, Tolls, Orcs, Ogres, etc.
- Undead including Zombies, Ghouls, Mummies, Skeletons etc.
The abilities and skills of the enemies can change in the later stages of the game. Slow moving Zombies can, therefore, mutate into fast moving Battle Zombies which are not only very well armed but also extremely dangerous.
Mummies have magical abilities, allowing them to change form and become volatile sandstorms, while the Liches which are made of pure energy tend to feed on the energies of the character. Ghosts send out bolts of lightning and can turn players' followers into ice and stone.
Other enemies can also change, meaning they can appear in one form, and morph into another form (like the Werewolf), or become something else once they have died.
The foes can attack in the following ways:
- Close combat with natural weapons, biting and claws
- Close combat with standard weapons
- Ranged combat with bows, spears or natural 'spit' or 'breath' attacks
- Magical attacks that can either be area, target or attribute attacks
- Special attacks defined by their species (like the 'ram' attack of the Minotaur)
Enemies don't just stand around and wait for the hero to slay them. Instead they will use tactics, either grouping together and attacking the character in numbers or fleeing when the situation seems hopeless. Even the same type of enemy can employ different tactics and weapons.

- Story NPCs bestow story relevant quests. They can also become companions who will fight alongside the character.
- Secondary NPCs offer numerous secondary quests. The character will be treated with much more respect by the Story NPCs with each completed secondary quest. If the character has completed all secondary quests as well as story relevant quests in a region, the region will then become cleared of evil.
- Special NPCs like the Trader, Blacksmith or Master have interactive menus:
- The Trader: Players can buy and sell equipment and treasures
- The Blacksmith: Can uniquely modify weapons for the player to give them special attributes
- The Master: Teaches the player new skills and special moves
